Your appeal will be carefully reviewed and a response will take at least two (2) weeks. A temporary clearance can be issued at the Circulation Desk once you have submitted this completed form. The clearance is only valid at the Circulation Desk. It will not allow you to request or renew books online and it does not clear blocks with other University offices.

All appeals must be made within one year of billing date.
